Levomilnacipran, sold under the brand name Fetzima, is an antidepressant, used for the treatment of major depressive disorder in adults.[4] It is the levorotatory enantiomer of milnacipran, and has similar effects and pharmacology, acting as a serotonin–norepinephrine reuptake inhibitor.[7][8]
Levomilnacipran was approved for medical use in the United States in July 2013.[9]
Medical uses

Levomilnacipran is indicated for the treatment of major depressive disorder in adults.[4]
Side effects
Side effects seen more often with levomilnacipran than with placebo in clinical trials included nausea, dizziness, sweating, constipation, insomnia, increased heart rate and blood pressure, urinary hesitancy, erectile dysfunction and delayed ejaculation in males, vomiting, tachycardia, and palpitations.[10][11]
Pharmacology
Pharmacodynamics
Relative to other serotonin and norepinephrine reuptake inhibitors, levomilnacipran, as well as milnacipran, differ in that they are much more balanced reuptake inhibitors of serotonin and norepinephrine.[12][13][14] To demonstrate, the serotonin:norepinephrine ratios of SNRIs are as follows: venlafaxine = 30:1, duloxetine = 10:1, desvenlafaxine = 14:1, milnacipran = 1.6:1, and levomilnacipran = 1:2.[12] The clinical implications of more balanced elevations of serotonin and norepinephrine are unclear,[12] but may include improved effectiveness, though also increased side effects.[13][14][15]
Levomilnacipran is selective for the serotonin and norepinephrine transporters, lacking significant affinity for over 23 off-target sites.[16] However, it does show some affinity for the dizocilpine (MK-801/PCPTooltip phencyclidine) site of the NMDA receptor (Ki = 1.7 μM), and has been found to inhibit NR2A and NR2B subunit-containing NMDA receptors with respective IC50 values of 5.62 and 4.57 μM.[16] As such, levomilnacipran is an NMDA receptor antagonist at high concentrations.

Levomilnacipran: Baseline Overview
Levomilnacipran, formally designated as (1S,2R)-milnacipran, is the purified levo-enantiomer of the racemic compound milnacipran and is a potent and selective serotonin-norepinephrine reuptake inhibitor (SNRI) [1]. It is the active pharmaceutical ingredient (API) in the FDA-approved extended-release formulation Fetzima® [2]. Its mechanism of action is defined by a dual inhibition of the human norepinephrine transporter (NET) and serotonin transporter (SERT) with a notable preference for norepinephrine reuptake inhibition over serotonin [1]. Procured as a high-purity chemical or reference standard, it is essential for analytical method validation, pharmacological research, and the development of generic antidepressant formulations.
Levomilnacipran: Why Generic Substitution Fails
Levomilnacipran cannot be considered a therapeutically or pharmacologically equivalent substitute for other serotonin-norepinephrine reuptake inhibitors (SNRIs) or its racemic parent, milnacipran. While all drugs in the SNRI class share a common mechanism, they exhibit significant quantitative differences in their potency and selectivity for the norepinephrine transporter (NET) relative to the serotonin transporter (SERT) [1]. As detailed in the evidence guide below, levomilnacipran is distinguished by a unique 1:2 SERT-to-NET potency ratio, the highest among all marketed SNRIs, and achieves simultaneous dual reuptake inhibition across all clinically relevant doses, a profile not shared by venlafaxine or duloxetine [2]. Its favorable pharmacokinetic profile and low protein binding further distinguish it from alternatives, making direct substitution without adjusting for these quantifiable differences unreliable in both research and clinical contexts.
Levomilnacipran: Comparative Differentiation Evidence
NET Selectivity Advantage
Levomilnacipran demonstrates a significantly higher selectivity for inhibiting the norepinephrine transporter (NET) over the serotonin transporter (SERT) compared to the SNRIs venlafaxine and duloxetine. In a direct comparative in vitro study using human recombinant transporters, levomilnacipran’s NET selectivity was 17-fold greater than that of venlafaxine and 27-fold greater than that of duloxetine [1]. Furthermore, levomilnacipran has a consistent 1:2 (SERT:NET) inhibition ratio across its entire dose range, whereas venlafaxine and duloxetine exhibit sequential, dose-dependent effects, acting primarily as SSRIs at lower doses and only achieving SNRI activity at higher doses [2].
Transporter Binding Affinity & Potency
Levomilnacipran exhibits high binding affinity and functional potency at human serotonin (SERT) and norepinephrine (NET) transporters. Its binding affinity (Ki) is 91-92.2 nM for NET and 11-11.2 nM for SERT, with functional reuptake inhibition (IC50) values of 10.5-11 nM for NET and 16-19 nM for SERT [1][2]. This contrasts with the Ki values reported for its racemic mixture, milnacipran, where one study found Ki values of 200 nM for NET and 123 nM for SERT, indicating levomilnacipran has significantly greater affinity, especially for NET [3]. This high potency translates into measurable, dose-dependent increases in cortical extracellular levels of both neurotransmitters in vivo, with minimal effective doses (MEDs) of 10 mg/kg for norepinephrine and 20 mg/kg for serotonin in rat models [1].
Description
Levomilnacipran is a selective serotonin and norepinephrine reuptake inhibitor (SNRI), although it is a more potent inhibitor of norepinephrine reuptake than serotonin reuptake. Levomilnacipran is the more active 1S,2R-enantiomer in the racemate [milnacipran]. Once administered, interconversion between levomilnacipran and its stereoisomer does not occur in humans. First approved by the FDA on July 25, 2013, levomilnacipran is used to treat major depressive disorder in adults. While levomilnacipran was previously investigated and proposed as a potential treatment for stroke in Europe, the EMA decided against this use.
Levomilnacipran is a Serotonin and Norepinephrine Reuptake Inhibitor. The mechanism of action of levomilnacipran is as a Norepinephrine Uptake Inhibitor, and Serotonin Uptake Inhibitor.
LEVOMILNACIPRAN is a small molecule drug with a maximum clinical trial phase of IV (across all indications) that was first approved in 2013 and is indicated for major depressive disorder and fibromyalgia and has 3 investigational indications. This drug has a black box warning from the FDA.

Pharmacological Mechanisms of Action
Dual Serotonin-Norepinephrine Reuptake Inhibition Dynamics
Levomilnacipran (LVM) is a potent dual serotonin-norepinephrine reuptake inhibitor (SNRI) distinguished by its preferential affinity for the norepinephrine transporter (NET) over the serotonin transporter (SERT). This selectivity profile differentiates it from other SNRIs and underpins its unique pharmacological activity.
Relative Potency Ratios (NE:5-HT) in Synaptic Transporter Binding
LVM exhibits a 2:1 potency ratio for NET versus SERT inhibition, as quantified through in vitro assays using human recombinant transporters. Key binding affinity values include:
Despite the lower Ki value indicating higher SERT affinity, functional reuptake inhibition assays reveal greater potency for NET. Half-maximal inhibitory concentrations (IC50) are:
- NET IC50 = 10.5 nM
- SERT IC50 = 19.0 nMThis yields a NET/SERT potency ratio of 0.6 (or 1:2 when expressed as NE:5-HT selectivity) [3] [8].
Table 1: Comparative Transporter Affinity and Potency of SNRIs [4] [8]
| Compound | NET Ki (nM) | SERT Ki (nM) | NE:5-HT Potency Ratio |
|---|---|---|---|
| Levomilnacipran | 92.2 | 11.2 | 1:2 |
| Milnacipran | 200 | 123 | 1:1.6 |
| Duloxetine | 8.9 | 0.2 | 1:10 |
| Venlafaxine | >10,000 | 17.9 | 1:30 |
Dose-Dependent Modulation of Monoaminergic Selectivity
LVM’s monoaminergic effects are dose-dependent. At lower doses (e.g., 10 mg/kg in rodents), it preferentially increases extracellular norepinephrine (NE) levels in the prefrontal cortex. At higher doses (≥20 mg/kg), it achieves balanced NE and serotonin (5-HT) reuptake inhibition:
This contrasts with other SNRIs (e.g., venlafaxine), which require doses >150 mg to significantly inhibit NET [2] [8].
Table 2: Dose-Dependent Reuptake Inhibition Profile of Levomilnacipran [2] [7] [8]
| Daily Dose (mg) | NET Inhibition (%) | SERT Inhibition (%) | Dominant Activity |
|---|---|---|---|
| 20 | ~70% | ~50% | Noradrenergic |
| 40–80 | ≥90% | 70–80% | Balanced |
| ≥120 | >95% | >85% | Balanced (slight NE bias) |
Receptor Binding Profiles and Off-Target Affinities
Lack of Muscarinic, Histaminergic, or Adrenergic Receptor Interactions
A defining feature of LVM is its high target specificity. Screening against 23 neurotransmitter receptors, ion channels, and transporters confirmed:
- No significant affinity for muscarinic acetylcholine receptors (IC50 > 100,000 nM) [1] [3]
- Negligible binding to histamine H1 receptors (IC50 > 100,000 nM) [1]
- Minimal interaction with α1-, α2-, or β-adrenergic receptors (IC50 > 10,000 nM) [3] [8]
This clean off-target profile reduces side effects associated with non-selective antidepressants (e.g., sedation, orthostatic hypotension) [1].
NMDA Receptor Antagonism at Supra-Clinical Concentrations
At supra-therapeutic concentrations (>1 µM), LVM acts as an NMDA receptor antagonist. Key findings include:
- Binds the dizocilpine (MK-801) site on NMDA receptors (Ki = 1.7 µM) [5]
- Inhibits NR2A subunit-containing NMDA receptors (IC50 = 5.62 µM) [8]
- Inhibits NR2B subunit-containing NMDA receptors (IC50 = 4.57 µM) [8]
This activity occurs at concentrations 10–50 times higher than therapeutic plasma levels (Cmax ≈ 341 ng/mL or 1.38 µM at 120 mg/day) [1] [6]. While irrelevant for antidepressant effects, it may contribute to neuroprotective effects in non-clinical models [5].
Table 3: NMDA Receptor Inhibition by Levomilnacipran [5] [8]
| NMDA Receptor Subtype | IC50 (µM) | Clinical Relevance |
|---|---|---|
| NR2A | 5.62 | Supra-therapeutic (>3× Cmax at 120 mg) |
| NR2B | 4.57 | Supra-therapeutic (>3× Cmax at 120 mg) |
| MK-801 binding site | Ki = 1.7 | Supra-therapeutic (>1× Cmax at 120 mg) |
